A new inquiry has been ordered into the fire service involved in the attempt to rescue Alison Hume, who fell down a disused mineshaft in Galston, Ayrshire.
Scottish Conservative MSP for Ayr, John Scott said:
"The death of Alison Hume is desperately tragic and made worse by the fact that if better operating procedures had been in place she could have been saved.
"It is important that lessons are learned from this incident, as such an incident must not happen again. In the meantime I believe an apology from Strathclyde Fire and Rescue service would be welcomed, particularly by the Hume family."
